Parental Consent and Release Form:

I, the lawful parent and guardian of (child) hereby grant consent for my child to participate in   summer camp activity of McNicholas High School and authorize the staff of McNicholas High School to act for me according to their best judgment in any emergency requiring medical attention, and I hereby waive and release the camp from any and all liability for any injuries or illnesses incurred while at camp. I have no knowledge of any physical impairment that would be affected by the named camper’s participation in the camp.

I give permission for McNicholas High School to use my child’s picture on the McNicholas website or in any publication that promotes the school.

Completing and submitting this form acknowledges that I fully understand this statement and its content.
